Unwaith y byddwn wedi derbyn atgyfeiriad, bydd gennym gyfarfod t\u00eem amlddisgyblaethol i drafod addasrwydd ar gyfer ein hysbyty. Os byddwn yn ystyried bod y claf yn addas, byddwn yn ceisio ei asesu o fewn 48 awr. Bydd y t\u00eem asesu yn cynnwys; Seiciatrydd Ymgynghorol, staff nyrsio cofrestredig ac ar adegau bydd yn cynnwys seicolegydd ymgynghorol a\/neu therapydd galwedigaethol. Yn dilyn yr asesiad, bydd penderfyniad yn cael ei wneud, bydd y sawl a wnaeth yr atgyfeirio yn cael ei hysbysu a bydd adroddiad yn cael ei ddarparu ar gyfer cais am arian. <\/pee><\/div>\n
